Our Preventive Plus Dental Plan, which is available exclusively in our dental office, is intended to cover your all basic dental care needs. This is a yearly reduced fee dental plan that allows patients and their families to enjoy quality dental care at a discounted rate of 10% off the office’s usual. We also ask for customary and affordable charges for restorative and major dental procedures. This plan was created especially for patients who don’t have dental insurance or whose employer no longer offers it. We developed this strategy to help you in your efforts because regular dental care is an important element of your overall health. It offers several amazing features which are really exciting! • There is no annual limit.
• There is no deductible.
• There are no waiting periods—get started on your therapy right away!
• There is no need for preauthorization or claim forms.
• There are no limitations based on pre-existing conditions
• There is no age limit.
• The effective date is the day on which the patient signs up, and the renewal date is the same every year.
• This is NOT dental insurance, but rather a dental discount plan.
• This plan CANNOT be used along with any other dental insurance plan.
• Palatine Dental Associates is the only place where you can use this plan. This discount will not be offered by any of the referred professionals.
• This plan can be canceled at any time by submitting a written request. However, this dental discount plan is non-refundable.
• The plans and fees are subject to change on an annual basis.
